When a business telephone call is placed (and the call does NOT go to a receptionist, but directly to the correct extension), then who must speak first to introduce oneself? Is it the caller or the callee?

You mention of a "Yo", but I have had worse situations - sometimes it is very silent - that is the person picks up and then expects me (the caller) to know the phone has been picked up... that sucks...
