to get reimbursed by fsa, you need to submit a credit card receipt and a prescription or note from your doctor prescribing massage therapy as treatment. as year end approaches, i've been doing this for the past several years to use my remaining fsa. i've been to many a amp using my fsa..i pay the basic service on my cc and tip in cash...place doesn't have to be licensed. you fill in the form supplied by your insurance carrier and hobby away. i already put next years massage money away in fsa....don't leave home without your doctors note or prescription. could come in handy if the place is raided. you were there for medical treatment...lol
